  • [S1654] "Miss Waicelunas, Edward F. Hardy Wec February 8," The Helena Daily Independent,25 Feb 1941page 10; Fresno County Public Library, Sanger Branch Library. This was a weekly newspaper. • [S1777] "Official Military Personnel File (OMPF), David E Johnson, Serial Number 33 675 219 (enlisted), 0-2065286 (officer) (US Army and US Air Force Reserve, Army Active (Enlisted) Induction Date: 26 Mar 1943)"; National Personnel Records Center (National Archives); 1 Archives Drive, St Louis, Missouri. About 75% if the records for veterans of the US Army (and Army Air Corps) during World War I and World War II (and the period between these wars) were lost due to a large fire in this facilty in 1973. However, this loss did not affect US Navy and US Marine Corps records.

The file was received in April 2017 from the National Archives branch in St Louis, Missouri under Request Number 2-20276462065. The file contained 202 pages. It was re-ordered in approximately chronological sequence (exceptions to chronological included the cover page and separation papers; the separation papers are placed near the beginning of the revised sequence because they provide a useful summary of much of the soldier's military career). A second set of handwritten numbers were placed on each page to "capture" this approximate chronological sequence. The page numbers corresponding to the approximate sequence are enclosed in a hand-drawn approximate rectangle. Unless indicated otherwise, the page numbers in what follows are the "chronological" ones, the ones enclosed in an approximate rectangle. Three pages of notes by CCF have been added to the end of the material original in the file. In addition, several pages of information gleaned for e-mail exchanges among CCF and various Johnson family members have been added following the CCF notes. These provide additional information about Johnson's life and military career.
There are several duplicate (and/or near-duplicate) items in the file.
The general types of documents in each group of pages is as follows:
Page 1 - Cover letter from NARA
    Pages 2-4 - Separation Papers (1945)
    Page 5 - Photograph (undated)
    Pages 6-9 - Initial (1943) enlistment
Pages 10-21 - Misc. forms and questionnaires (1943-1944)
Pages 22-39 - Service record "booklet" for enlisted service as a (recruit) private and an aviation cadet in 1943 and 1944, prior to being commissioned. Looks like each page of the booklet was about 3 inches wide and 8 inches high. Booklet initially had 24 numbered pages, but several additional pages were inserted (by means of staples, apparently, but one or more may have been glued). When the booklet was laid flat, two adjacent pages could be copied in a single image when no inserts were attached to either page. In order to obtain images of both sides of the inserted pages, the number of pages in the copy is greatly in excess of the number that would have been used to copy the "24" (if there had been no inserts). The information in the booklet was arranged by topic (vaccinations, training, duty stations, promotions, awards, etc.), and then chronologically (or approximately so) within the topic.
Pages 40-43 - Various orders associated with transition from aviation cadet to commissioning
Page 44 - Oath of office, associated with commission
Page 45 - Bombardier School final grade sheet
Pages 46-51 - Special orders issued at conclusion of training (21 and 22 July 1944 - the later one seems to "undo" some of the provisions of the earlier one with respect to time off)
Pages 52-53 - Appointment letter, in lieu of a formal commission document
Page 54 - Documents discharge from enlistment to accept commission
Page 55 - Misc training documentation
Pages 56-58 - Documentation for three Air Medals (Air Medal with two Oak Leaf Clusters)
    Pages 59-60 - Efficiency report 18 Mar 1945 (covering last ½ of 1944)
Page 61 - Promotion to First Lt.
Pages 62-98 - Various documents associated with return to US from service in Europe, separation from active duty, and appointment as reserve officer. Includes request for award of Distinguished Flying Cross (DFC) (pages 79 and 80)
Pages 99-100 - Award and delivery of DFC (awarded on 1 Dec 1945, after separation on 30 Sept 1944)
Pages 101-180 - Misc documents dealing with service in Air Force reserves until 1953. Contains several summaries of military and civilian experience.
Pages 181-184 - Information added after his death (burial benefits, etc.). Includes informal notation apparently asking about possible Purple Heart Medal.
Pages 185-199 - Scans of various Morning Report Location Cards
Pages 200-202 - Misfiled papers intended for different men named David Johnson.
Only selected pages are included in the set of images attached to the (digital) web (or internet) and TMG versions of this material. Duplicate pages and pages judged to contain repetitive or "uninteresting" material have been eliminated.
