The image of the completed form shows that the Western Pennsylvania District Court had a special rubber stamp which apparently gave the full title of the monarch of England (a portion of the process was for the applicant to state that he or she intended to renounce allegence to this monarch). The title includes "defender of the faith, emperor of India", etc.

  • [S1782] "Official Military Personnel File (OMPF), Robert William Smith, Serial Number 924-57-03 (US Navy, Enlisted 27 May 1944)"; National Personnel Records Center (National Archives); 1 Archives Drive, St Louis, Missouri. About 75% if the records for veterans of the US Army (and Army Air Corps) during World War I and World War II (and the period between these wars) were lost due to a large fire in this facilty in 1973. However, this loss did not affect US Navy and US Marine Corps records.

The file was received from the National Archives branch in St Louis, Missouri in response to Request number 2-20092514438. It contains 119 pages and appears to be fairly complete. The pages (including the transmittal letter written in 2016) were initially numbered to record the sequence in which they were received, which is not chronological or reverse chronological, although the page initially given the number 2 (the first page of the 1940-era file, following the 2016 transmittal letter) appears to be a "separation paper" (which would among the last pages generated). The entire file was then scanned. The file contains several duplicate items, some items that are obvious errors (probably typographical), and some puzzling inconsistencies. It appears that some of the original pages had information on both sides of a sheet, and both sides were copied (even though, in some cases, little or no information specific to the individual was on the reverse side).

Interesting items (keyed to the handwritten page numbers):

    Page 1: Transmittal letter.

Pages 2 and 3: 2 Feb 1946, Separation Papers (the two pages are identical) issued at Great Lakes Separation Center, Great Lakes, Illinois. At time of separation he was a "Fireman, 1st Class, SV-6, USRN". The V-6 indicates General Service and Specialists in the US Naval Reserve. It is unclear what the "S" is for. USNR is United States Navy Reserves.

The remainder of the sheets in file are in the following general catetories:

    Leave -- Pages 26, 47, 49, and 70

    Induction: Pages 4. 16. 21-23, and 33.

    Physical Exams: pages 9, 17-20, 24-25,34, 97-109, and 111-119.

    Insurance: Pages 29-32, 95, and 96.

    Transfers: Pages 5.7, 8, 10-13, 28. 38.41-43,51, 53-60, 69, 72, 79-80, 82, 83, ad 95.

    Training: Pages 35-37, 39, 48, 81, and 86-92.

    Promotions: Pages 27, 40, and 84.

    Discharge Related: Pages 6, 14, 15, 62-67, 93, 94, 110.

    Awards/Service: 44-46, 50, 52, 61, 68, 73, 74-78.

For this file, the pages were not re-sorted into chronological order and then re-numbered.

The file contains duplicate and multiple copies of several item. In some cases, the original document apparenlty contained information on both sides of the page. These were scanned onto two separate sheets by the National Archives.

Digital images of selected pages are included as exhibits to illustrate this source. Several of the pages that are not included are either at least partial duplicates of other pages or else contain little pertinent information.

Miguel Malvar is the current (as this is written in June 2017) name of a Philippine Navy corvette. The current (2017) Miguel Malvar was USS PCER-852 in 1945, where "PCER" indicates "Patrol Craft Escort - Rescue". Later, in 1956, it was re-designated USS Brattleboro (EPCER-852). It was later transferred to the Republic of Viet Nam (South Viet Nam), in 1966. It escaped to the Philippines when North Viet Nam took over South Viet Nam in 1975 and was later formally transferred to the Philippine Navy. As of 2017, it is one of the oldest still-active warships in the world. The history of the ship while it was operated by the US Navy is described in the "BRP Miguel Malvar (PS-19)" Wikipedia article.

Additional public housing units were constructed in the vicinity of the Cabrini Homes developement, incluidng high-rise units beginning in the 1950s. An adjacent development was named for a union leader whose surname was Green, so that the overall complex became known as Cabrini-Green.

By the 1970s, the area had become a very undesirable slum, with high crime and criminal gang activities. In the late 1990s, the area began to be demolished (especially the high rise units). It appears that (as of 2017) the only units that are still standing are some number of the original 1942 row houses. As on July2017, Google Street View contained views of several of these buildings, including 510 W. Locust (athough, street addresses and unit numbers were not visible in the photos, so it was not clear exactly which building or unit was Number 510). These Street View photos (appraently taken in 2015) showed the units were surrounded by "cyclone"-type fencing and appeared to be "boarded up" and unoccupied. It is not known if this "boarded up" condition was in preparation for refurbishment or demolition. However, the 2015 Street View photos appear to show that some units only a block or so away have been refurbished (and look quite neat). So perhaps the plan is to refurbish the entire area.
